Appointments

If you would like to make an appointment, please contact our office by phone or email. Our office facilitates communications and emergency calls and appointments are available and welcome, especially for new patients seeking help.

Cancellation

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.

Payment Policies

We are preferred providers for most major insurance plans. We will be happy to submit all insurance forms for you and help you recover the most from your benefits. We do our best to provide affordable options for the treatment you need. Forms of payment accepted are check, cash, or Visa and Mastercard.

Insurance and Billing

We accept most traditional insurance plans. We are in Network with Aetna, Ambetter, Amerigroup, Blue Cross Blue Shield, Cigna, First Choice, Lifewise, Kaiser, Medicare, Moda Health, Premera, Providence, Regence, and United Healthcare and more. We are a participating provider for most Medicare Advantage and Supplemental plans. Please contact our office to verify acceptance of your particular plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.
